About the matter you apply the certification document for foreign people at Narita city office.
I asked Narita city office about it, then they said that they open the office until 19:00 on Tuesday.
There might be no person who can talk in English, but office staff can handle with the paper which you previously prepare for if you can.
The office staff might not be able to talk in English, but they can read English sentences you will prepare.
You will write the sentence below in English.
I apply 1 the certification document for foregin people. This is for opening bank account.
You have to write application form, so I will send the sample to your home that I’ve already got it from Narita city office.
Please write application form with referring the sample at city office.

こんにちは。
成田市役所で外国人登録記載事項証明書を申請する件ですが、
成田市役所に問い合わせたところ、火曜日は夜7時まで窓口で業務しているそうです。
英語が話せる職員が常駐しているわけではありませんが、あらかじめ何が必要なのかを紙に書いてきていただければ、それを見て職員が対応するとのことです。
英会話は無理でも、紙に書かれた英語ならば理解できるだろう、ということです。
紙に、「外国人登録記載事項証明書を1通申請します。用途は銀行口座開設のため」と書けばOKです。
窓口で、交付申請書を書かなければならないんですが、
その記入要領については、私が成田市役所から取り寄せたサンプルがありますので、それをSteveさんの自宅へ郵送します。
それを見て、窓口で交付申請書を書いてください。
